At Revivify we may collect your personal data for client service purposes only, is it allows us to contact you via email or text. Contests, marketing, communications, campaigns, and other promotions - with your consent - we may use your personal data to provide you with information about goods or services. This may be done via email, ads, SMS, phone calls, and postal mailings to the extent permitted by applicable laws. The use of personal data is voluntary, meaning you can oppose/withdraw consent at any time. Revivify will never share your information with any other party without written consent from you. 

As your Massage Therapist, I do not diagnose medical conditions and I cannot provide prescriptions. Massage Therapy is not a substitute for a medical evaluation or treatment.
